Our supported living service at Ashrey Care provides safe, high‑quality accommodation for adults who require personalised support to live as independently as possible. We work with individuals aged 18 and over who may have learning disabilities, autism, mental health needs, physical disabilities, or complex support requirements.
Each referral is assessed carefully to ensure we can meet the person’s needs and offer a home where they can thrive.
Who We Support
We welcome referrals for adults who:
- Require support to develop daily living skills
- Benefit from structured routines and personalised care
- Need help to access the community safely
- May present with behaviours that challenge
- Require support with emotional regulation or mental health
- Are transitioning from residential care, hospital settings, or family homes
- Are seeking long‑term, stable accommodation with tailored support
We consider each referral individually, focusing on compatibility, safety, and the person’s long-term goals.
Our Referral Process
We aim to make the referral process clear, responsive, and collaborative.
Referrals can be made by:
- Local authorities
- Social workers
- NHS professionals
- Families or advocates
- Self‑referrals (where appropriate)
We gather initial information to understand the person’s needs, preferences, and current situation.
We request relevant documents such as:
- Care plans
- Risk assessments
- Behavioural support plans
- Health and medical information
- Education or placement reports
- Any legal frameworks (e.g., DoLS, guardianship, court orders)
This helps us build a full picture of the individual’s support requirements.
Assessment & Compatibility Review
Our team conducts a holistic assessment, which may include:
- A face-to-face or virtual meeting with the individual
- Discussions with family, professionals, and advocates
- Environmental and compatibility assessments with existing tenants
- Evaluation of risks, support needs, and required staffing levels
We ensure the placement is safe, appropriate, and aligned with the person’s goals.
Offer of Placement
If the assessment confirms suitability, we provide:
- A proposed support plan
- Indicative staffing model
- Accommodation details
- Transition plan and timelines
- Costings for local authority approval
We work collaboratively to agree on next steps.
Transition & Move-In
Transitions are tailored to the individual and may include:
- Introductory visits
- Overnight stays
- Gradual familiarisation with staff and routines
- A personalised welcome plan
